Gundersen Lutheran Medical Foundation provides training materials for care professionals who wish to improve the quality of advance care planning within institutions and communities. Our resources are designed for use by professionals from varied backgrounds who may be called on to assist individuals in making decisions regarding their future medical care, including end-of-life decisions.
