Promoting Excellence : Special Opportunity Grants : The National Advanced Illness Coordinated Care Council

Project Overview
The goal of the National Advanced Illness Coordinated Care Council was to promote peaceful dying and to bridge the gap between good advanced illness/end-of-life care and its practice. The NAICC accomplished this goal by developing, implementing and evaluating practical, affordable and immediately usable advanced illness coordinated care (AICCC) programs within health systems and health plans. The AICCC is affiliated with the Life Institute, VA Health Care Network, Upstate NY and Dartmouth Medical School.

AICCC programs use a nurse or medical social worker trained in standard models to support the physician and to provide psychosocial support for patients facing advanced illness and their families. The AICCC program is a six-visit intervention of structured conversations and specialized care coordination for patients with advanced illness and their families.
