A 26-item question guide was designed to be used in a face-to-face home interview
of parents two months after the patient has died. The 26 items are focused
in general on consumer/patient preferences being met in the following areas:
place of death, communication, treatment and care, pain and symptom management,
religious/spiritual/cultural beliefs, and preparation for death.
