Definition:
The document describes an administrative policy developed and used by Regence
BlueShield on an ongoing basis as a result of the Pediatric Palliative Care
Project. It provides guidelines to health plan case managers in allowing
a flexible use of benefits to provide palliative care services to those with
life-limiting conditions who might fall in the gap between eligibility for
home health care or hospice.

How the grantee used this instrument:
Regence is one of the health plans that have a formal provision for palliative
care, resulting from the research project, that provides coverage for palliative
care services. Children (with health plan approval) can be referred to a hospice
agency that will provide them with palliative care case management, care coordination,
medical oversight, and psychosocial support in the home, reimbursed by their
health plan.
