How the grantee used this instrument:
The directory is distributed through the National Hospice & Palliative Care
Organization's - Continuing GRACE Correctional End of Life Care initiative to
all current and under-development end of life care programs, Department of Corrections
in all states, and the Bureau of Prisons. It was designed to encourage dialogue
and interaction between programs.
