Description:
Three self-assessment tools for use by organizations in rating their perceptions
of effectiveness in meeting the stated objectives or outcomes, and identify
barriers that inhibit services. The first tool is a complete assessment, the
second tool is a shorter version of the first, and the third tool is used to
evaluate cross-cultural system support in the organization.

How the grantee used this instrument:
These questionnaires were modified from the City of Hope Tools. They were used
to self-assess supportive structures, to build on existing data, and to be congruent
with JCAHO standards. Designed to be administered in a group discussions, they
require 2 - 4 hours to complete preferably in 2 different sessions.
