Hoping to enable patients to make advance care plans long before they arrived in an acute care setting, Community Hospitals of Indianapolis developed a four-hour training program for parish nurses and pastors designed to:

Previous attendees receive information about upcoming sessions. The group has also developed a resource guide on end-of-life issues for parish nurses and pastors.
